Responsibilities
- Provide nursing care to patients in hospice care in patient homes and facilities
- Develop and implement individualized care plans for patients and their families
- Coordinate with interdisciplinary team members to provide comprehensive care to patients
- Administer medications and treatments as prescribed
- Monitor and assess patient symptoms and provide comfort measures as needed
- Educate patients and their families on end-of-life care and support
- Document patient care and progress in electronic medical records
- Provide emotional and spiritual support to patients and their families
- Advocate for patients and their families to ensure that their needs are met
- Participate in on-call rotation as needed
- Active RN license in the state of practice
