At Trinity Health At Home, we are driven by a shared mission to serve with compassion and excellence. We’re seeking a Home Health Clinical Educator to support and guide our nursing and therapy teams through orientation, ongoing training, and clinical excellence initiatives.

Key Responsibilities

• Develop and deliver onboarding and continuing education programs
• Serve as a mentor and clinical resource for new and current staff
• Ensure compliance with CMS, accreditation, and internal standards
• Evaluate clinical performance and support quality improvement
• Lead initiatives to elevate care delivery and patient outcomes
• Partner with leadership and staff to identify training needs
  
  

✅ Qualifications

• RN license in the State of Michigan required
• Bachelor's degree in nursing or related field (master’s preferred)
• Minimum 2 years of home health clinical experience
• 1+ years of experience in a teaching, training, or educator role
• Strong knowledge of Medicare regulations and OASIS preferred
• Excellent communication, leadership, and teaching skills
• Must have valid driver’s license and reliable transportation
